Join Matt as he sits down with Jenny Fryer, Project Development Manager, and Angela Musson, Project Support Officer at the Lincolnshire Integrated Care Board (ICB), for a deep dive into the vital work surrounding ReSPECT - the Recommended Summary Plan for Emergency Care and Treatment.
Together, they explore how this personalised emergency care plan is helping ensure people receive the support they need, exactly when they need it, especially during those critical and often unexpected moments.

Matt is joined by Lee Vaughan to discuss chronic pain.
Lee Vaughan has lived with chronic pain since his teens. After attending a pain management education programme, he went from taking 28 pills a day to just one, and developed a new understanding of the causes of his pain and how to manage it.
Now an advocate for this kind of support, Lee has trained as a health coach and runs an online group for people living with chronic pain.
Tune in to listen as Lee makes us think about how we understand persistent pain and shares his learnings and his experiences.

In this episode, we discuss the Grantham Joint Aches and Pains Hub event with Gemma Wright, Personalised Care Lead, K2 Primary Care Network, and Alison Wilkinson and Nicola Bussey, both Physiotherapists at United Lincolnshire Teaching Hospitals NHS Trust (ULTH).
Led by the K2 Healthcare Sleaford Primary Care Network (PCN) and the It's All About People Personalisation Programme team, the Grantham project brought together statutory, third sector and independent providers at a local leisure centre in the town to offer a ‘one-stop-shop’ for people experiencing joint pain.
The event allowed people to speak to various health and care professionals in an informal, non-clinical environment. Given the increasing pressures on primary care and local hospital services, this was intended to improve people’s access to professional advice and support.
Key to this discussion is the theme of working differently and how that feels for clinicians.

Alison and Matt join Shaun Harrison, Adult Social Care Manager at Lincolnshire County Council, to explore the Council's Maximising Independence programme.
We hear about strengths-based approaches, Sparks, 12th-century monks, and some really great examples of personalised ways of working.
Maximising Independence seeks to promote independence and reduce the reliance on long-term support services.
The Maximising Independence Team works with: vulnerable adults, adults with learning disabilities, autistic adults, adults with physical disabilities, adult frailty, and adults with acquired brain injuries, in various care settings and the community. They do this by enabling people to reach their full potential by maximizing their independence and quality of life, and assist practitioners to make informed decisions about support needs. Types of referrals include: assessment of life skills, development of life skills, community inclusion, and observation and evidence gathering.

Personalised cancer care is widely discussed, but what does it actually look like in practice?
Matt spoke to Lauren Gray, a Macmillan Senior Cancer Care Coordinator who bridges hospitals and community services to support people as treatment ends. She showed how personalised approaches are improving experiences, smoothing transitions, and supporting better outcomes across the cancer pathway.
For Lauren, personalisation isn’t a “nice to have” – it’s a practical, human way to steady people at vulnerable moments and ensure no one feels alone as they rebuild their lives after treatment.
